Individual Support Services

Disability support services tailored to your needs.

We offer a range of services to support people with disability to live safely, independently and confidently at home and in the community. Whether you need help with everyday tasks, support to stay socially connected or clinical care for more complex needs, we can work with you to design support that suits your life.

Everyday support services

Our everyday supports help make daily life easier, safer and more enjoyable. Depending on your needs, support may include:

Personal Care

Help with daily activities such as bathing or showering, grooming, continence care, medication, mobility support, exercise routines, assistance with eating

Meal Planning & Preparation

Support with planning meals, preparing food, writing grocery lists, grocery shopping, cooking

Respite Care

In-home respite care can provide carers with time to rest and recharge while ensuring the person they support continues to receive quality care

Household Support

Help around the home including sweeping, mopping and vacuuming, cleaning kitchen and bathrooms, dusting and tidying, making beds and changing linen, washing, ironing and putting away clothes, taking out the rubbish

Social and Community Participation

Support to stay active and connected through social outings, visits with friends and family, trips to cafes, clubs, galleries or community activities, walks and time outdoors, transport to appointments, shopping and community events, companionship and conversation

Clinical Care

Some people may need more than everyday support to continue living well at home. Our clinical care services provide professional health support for more complex care needs, delivered with compassion, skill and respect.

This may include support from registered nurses and allied health professionals such as physiotherapists, occupational therapists and podiatrists.

Clinical care may be right for you if you need support with:

  • Medication administration
  • Wound care and pressure injury prevention
  • Continence education and management
  • Diabetes education and management
  • Mobility and movement
  • Rehabilitation and exercise support
  • Ongoing assessment of changing health needs

Clinical care may include:

  • An initial assessment and review of your care needs
  • Recommendations tailored to your health and wellbeing goals
  • Regular or one-off consultations with our clinical professionals
  • Reviews to make sure your support continues to meet your needs
  • Coordination with other services involved in your care, where needed

A team-based approach

Our clinical team works alongside our broader disability support team to provide care that is coordinated, responsive and tailored to you.

Our dedicated team of Registered Nurses, Physiotherapists, and Occupational Therapists are here to support your health and wellbeing at home. We work together to provide care that is professional, personalised and focused on helping you live as independently as possible.

Fees and Funding

Depending on your circumstances, services may be funded through your NDIS plan or accessed as a private fee-for-service arrangement.

NDIS-Funded Supports

As a registered NDIS provider, we deliver supports in line with current NDIS pricing arrangements, available here. If your supports are funded through the NDIS, our team can help you understand what services may be available to you under your plan.

Private Services

If you are not eligible for the NDIS, are waiting for your funding to be approved, or would like to purchase additional support beyond your plan, private services may be available. This option offers flexibility and can be used to access a range of in-home and community-based supports, depending on your needs.
