Aged Care Services We Provide

CatholicCare provides a range of in-home support services designed to help older people remain safe, independent and connected while continuing to live at home.

Our services are available to people accessing government-funded aged care programs such as Support at Home or the Commonwealth Home Support Programme (CHSP), as well as those arranging private support directly.

The services below are grouped in line with the Australian Government’s service categories. Not all services are available under all funding streams, our team can help you understand what’s available based on your individual circumstances.

Clinical Supports

Clinical supports are specialised health services designed to maintain or improve your functional and cognitive abilities. These services are delivered by qualified health professionals and are tailored to your individual health needs, goals and circumstances.

provides professional nursing and clinical support for older people who may require ongoing health monitoring or more specialised care while continuing to live at home.

This may include support relating to:

  • Chronic health conditions
  • Medication management
  • Wound and skin care
  • Clinical monitoring and assessments
  • Continence management (clinical)
  • Recovery following illness or hospitalisation
  • Ongoing health and wellbeing support

Services are delivered by experienced Registered Nurses and are tailored to individual health needs, goals and circumstances.

Telehealth consultations are also available where appropriate.

Allied Health services focus on supporting mobility, safety, independence and overall wellbeing to help older people remain living safely and confidently at home.

This may include support relating to:

  • Mobility and falls prevention
  • Functional decline
  • Strength and balance
  • Equipment and assistive technology recommendations
  • Home modification recommendations
  • Improving independence with daily activities

Allied Health services may involve physiotherapists, occupational therapists, and other health professionals depending on individual needs and goals.

Prescribed supplementary dietary products and nutritional support where related to functional decline or clinical need. This service is available under Support at Home funding where prescribed by a health professional.

Under Support at Home, clinical supports are fully government funded and do not require participant contributions.

Independence Support

Independence support helps you manage activities of daily living and maintain the skills needed to live independently. These services support your ability to stay active, safe, and engaged in your community.

Assistance with daily personal care and hygiene needs, including support with showering, dressing, grooming, mobility and the self-administration of medication.

Personal care also includes non-clinical continence management, such as assistance with changing continence aids and accessing advice or supplies.

From 1 October 2026, personal care services will be fully funded under Support at Home, meaning no out-of-pocket cost for approved participants.

Support to remain connected and engaged with your community, social networks and daily activities. This may include:

  • Group and individual social support
  • Digital education and support to improve online literacy and participation
  • Accompanied activities and outings
  • Visiting and check-in services
  • Support to engage in cultural activities for people from diverse backgrounds and life experiences

In-home companionship and social support, including regular visits and conversation to reduce social isolation and support wellbeing.

Assistance getting to appointments, activities and community, including direct transport or indirect transport such as taxis.

Support with physical activity, strength and mobility to help maintain independence and overall wellbeing. This may include exercise programs, remedial massage (where prescribed as part of a treatment plan), and other therapeutic supports delivered by qualified practitioners.

Treatment programs aim to provide you with the skills and knowledge to manage your own condition and promote independent recovery where appropriate.

Flexible respite for carers and families, providing supervision and support for an older person by someone other than their usual carer. Respite can be delivered individually or in a group setting, in the home or in the community.

Support to build or maintain practical life skills that promote independence in daily living.

Assistance technology includes equipment, devices and aids that support independence and safety at home. Home modifications include changes to your home environment to support safe living, such as grab rails, ramps or bathroom modifications.

Assistive technology and home modifications are available through the Support at Home program’s Assistive Technology and Home Modifications Scheme, subject to assessment and approval. Private arrangements may also be available, just speak with our team.

Under Support at Home, participants may contribute towards the cost of independence support services based on their individual financial circumstances.

Everyday Living

Everyday living support helps keep your home in a safe and liveable state so you can continue to live independently. These services focus on essential household tasks.

General house cleaning, laundry, and housekeeping support to maintain a clean and safe home environment.

Assistance with grocery shopping and errands, including accompanied or unaccompanied supports.

Support with meals and nutrition, including in-home meal preparation and pre-prepared meal delivery services.

Essential light gardening including lawn mowing, pruning and yard clearance for safe access. Services relate to safety and accessibility rather than visual appeal.

General home maintenance and minor repairs, including essential repairs where the task is something you used to be able to do yourself, or where required to maintain safety (e.g. replacing lightbulbs, repairing broken door handles).

Under Support at Home, everyday living services require the highest level of participant contributions, reflecting the Australian Government’s approach to prioritising funding for health and independence-related services.
