What is Clinical Care Plus?
In addition to receiving in-home help for tasks such as cleaning, cooking, shopping, and gardening, many people also need professional clinical support - like regular physiotherapy, wound care, or help with medication - to continue living at home. If you're finding that you now need higher care, we recommend Clinical Care Plus to ensure you get the right support.
Clinical Care Plus is for:
- Meeting your more complex care needs and improving your quality of life
- Access to a wide range of health professionals, including nurses, physiotherapists, and occupational therapists
- Highest-quality care through regular check-ins and adjustments to your services as needed
Clinical Care Plus includes:
- A comprehensive assessment and review of your higher-care needs
- Consultations with our clinical professionals
- Health professionals involved in your ongoing services
- Specialist health services available at either a flat rate or hourly rate
Clinical Care Plus is provided in addition to the standard Care Management and Package Management you receive with your regular home care services.
How much does Clinical Care Plus cost?
Your initial comprehensive assessment for Clinical Care Plus takes a couple of hours and focuses on gaining an understanding of your clinical needs, to identify any additional supports or referrals you may require.
We then recommend ongoing bi-annual assessments (or sooner, if there's a significant change to your health or wellbeing) to ensure that your clinical goals are maintained over time. We will then arrange regular or one-off consultation services with your required health professional. This could include a Registered Nurse, Physiotherapist, or an Occupational Therapist.
